• 90951 – end-stage renal disease services
• 90989 – other dialysis procedures
• 90999 – unlisted dialysis procedure, inpatient or outpatient

Dialysis CPT Modifiers

These two modifiers are the updated CPT dialysis codes for 2023:

• 45 – end-stage renal disease
• 81 – hospital-based ERSD provider
• 310 – not vaccinated against COVID-19
• 311 – partly vaccinated against COVID-19
• 39 – indicates any other under-immunized status
